Patterns and Predictors of Sleep Quality in Taiwanese Pregnant Women.

PURPOSE:: This study examined the patterns and psychosocial predictors of sleep quality in Taiwanese pregnant women. STUDY DESIGN AND METHODS:: A cross-sectional, correlational study design was used. A convenience sample of 400 pregnant women from two urban hospitals in Taiwan participated in this study. Data were collected using six self-report measures: Pittsburgh Sleep Quality Index (PSQI), Edinburgh Prenatal Depression Scale, Perceived Stress Scale, State-Trait Anxiety Inventory, Attitude to Body Image Scale, and Interpersonal Support Evaluation List. RESULTS:: Our findings revealed that the prevalence of pregnancy-associated sleep disturbance (PSQI score >5) was 65.5%. The mean score of the global PSQI was 7.25 ± 3.43, indicating that participant pregnant women in general reported mild sleep disturbance. Trend analyses of PSQI score indicated a linear trend, with a gradual decline in the sleep quality as the pregnancy progressed. Sleep latency, habitual sleep efficiency, and subjective sleep quality scores differed significantly by trimesters. Stepwise multiple regression analyses revealed that the best subsets to predict sleep quality were prenatal depression, prepregnancy sleep quality, current body image, and week of gestation for Taiwanese pregnant women. CLINICAL IMPLICATIONS:: These findings offer evidence that healthcare professionals should provide sleep counseling to pregnant women, and incorporate depression and body image into a supportive model of sleep-related prenatal care. PURPOSE:: This study investigated potential associations between maternal depression and specific infant health conditions, smoking, and socioeconomic characteristics among mothers of high-risk infants during medical follow-up visits. STUDY DESIGN AND METHODS:: Cross-sectional self-report, and interview data were collected from 114 mothers of high-risk infants previously discharged from a neonatal intensive care unit. Mothers were assessed at a postdischarge clinic visit. RESULTS:: Prevalence rates for a Center for Epidemiological Studies-Depression Scale score ?16) suggestive of a depressive disorder exceeded 20% at all time points for this sample of predominantly low-income, minority-race mothers. A greater number of mothers who had infants with a highly visible illness (e.g., surgical necrotizing enterocolitis) were depressed. Depression was also associated with the presence of a household smoker, younger age, and less education and income. CLINICAL IMPLICATIONS:: Given the prevalence rates, routine screening for postpartum depression in mothers of high-risk infants is imperative, with particular attention to the infant’s health, smoking, and socioeconomic variables. Sexual dysfunction in men with spinal cord injury: a case-control study.

The objective of this study is to confirm that spinal cord injury (SCI) alters the neurological pathways of sexual function (SF) and to ascertain whether other differences exist among men with SCI compared with men without SCI (WSCI) and sexual dysfunction (SD). The method comprise a case study of 98 men with SCI and SD, and 89 men with WSCI and SD as controls. A questionnaire was administered to these men regarding their SF, anxiety, depression, self-esteem and quality of life. Additionally, a penile eco-Doppler study was performed after a prostaglandin E1 (PGE1) injection, in addition to neurophysiological tests of the innervation pathways of SF. The mean age of cases (39.35 years) was significantly lower than the control group (48.5 years). The SCI group had a higher frequency of ejaculatory disorders, and the WSCI group had a higher frequency of decreased sexual desire, erectile dysfunction (ED) and premature ejaculation. The WSCI group had greater anxiety and depression than the SCI group. The systolic velocities of both cavernous arteries were higher in the SCI group. Significant differences were observed in electromyography of the bulbocavernosus muscle, pudendal sensory thresholds, somatosensory potentials, genital sympathetic potentials and electromyography of the cavernous bodies after PGE1 injection.
